OPS actualiza a profesionales de la salud ante aumento de casos de sarampión

Washington, DC, 21 de marzo de 2024 (OPS)- La Organización Panamericana de la Salud (OPS) organizó un seminario web dirigido a personal médico, de enfermería, laboratorio y epidemiología para proporcionar una actualización sobre la clínica, la vigilancia y la vacunación del sarampión, ante el aumento de casos de esta enfermedad en las Américas.

En lo que va de 2024, hasta el 16 de marzo, se han notificado 92 casos confirmados en siete países – la gran mayoría en Estados Unidos y Canadá-, que representan un 28% más que el total de casos reportado en todo 2023.

“Los casos de sarampión aumentan en todas partes”, advirtió la doctora Gloria Rey, asesora regional en Inmunizaciones de la OPS. A nivel global, los casos notificados a la Organización Mundial de la Salud (OMS) se incrementaron un 79% en 2023 frente a 2022 y más países del mundo (51) experimentaron grandes brotes. Además, según Rey, este año hay un incremento temprano de casos, lo que sugiere que la situación podría intensificarse en los próximos meses.

A pesar de la existencia de una vacuna efectiva contra el sarampión, siguen ocurriendo casos y muertes por esta enfermedad en todo el mundo. En 2022 se estimaron 136.216 fallecimientos relacionadas con el sarampión, lo que representa un aumento del 43% en comparación con el año anterior.

En las Américas, existe el riesgo de recibir más casos y de que se generen brotes debido a la baja en la vacunación. En 2024, solo el 15% de los casos reportados estaban vacunados según los datos disponibles. Además, el 70% de los afectados tenían menos de 10 años y el 78% de los casos contrajeron la enfermedad fuera de sus países (importados) o estuvieron relacionados con esta importación.

Para Desiree Pastor, asesora regional en Inmunizaciones de la OPS, “la pandemia de COVID-19 exacerbó la tendencia descendente en la cobertura de vacunación contra el sarampión, que aún no se ha recuperado y se mantiene estancada”. Durante años, la cobertura con la primera dosis de la vacuna SRP o triple viral se mantuvo por encima del 90%, pero en 2019 cayó al 87%, alcanzando su punto más bajo en 2022 con un 85%, muy inferior al ideal recomendado por la OPS de al menos el 95% para prevenir brotes.

Como dato alentador, la doctora Pastor señaló que, a pesar de la pandemia, los países de la región lograron llevar a cabo campañas de vacunación, alcanzando a más de 36 millones de niños de 1 a 12 años entre 2021 y 2023.

Por su parte, la doctora Tracy Evans-Gilbert, experta en pediatría clínica y miembro de la Comisión Regional para el Monitoreo y Sostenibilidad de la Eliminación del sarampión, la rubéola y el Síndrome de la Rubéola Congénita (SRC) de las Américas, brindó una sesión informativa sobre los aspectos clínicos del sarampión y la rubéola, destacando sus diferencias con otras enfermedades como el dengue. Su presentación buscó familiarizar a los trabajadores de salud con el sarampión para que puedan reconocerlo, notificarlo y abordarlo oportunamente.

La doctora Evans-Gilbert recordó que el sarampión, además de fiebre y exantema, con frecuencia produce síntomas respiratorios como tos y coriza (secreción nasal), lo cual no se observa en el dengue. Además, el virus es altamente contagioso, con capacidad para infectar a entre 12 y 18 personas por cada caso. Se transmite a través de gotitas respiratorias cuando alguien infectado habla, tose o estornuda, y puede permanecer activo en el aire hasta por 2 horas, propagándose en un radio de 2 a 4 metros.

Ante la llegada eventual de más casos importados de sarampión a la región, las asesoras de la OPS resaltaron la importancia de implementar medidas esenciales de prevención y control. Esto incluye la vacunación en áreas de alto riesgo lo antes posible, una vigilancia epidemiológica mejorada, que incluya búsquedas activas de casos en el terreno y foco en zonas como municipios fronterizos, y una respuesta rápida a brotes, que requiere equipos capacitados y protocolos para identificar y manejar casos en forma oportuna.

La OPS emitió en enero pasado una alerta instando a los países de las Américas a renovar la vigilancia y aumentar los esfuerzos de vacunación, ante el aumento de casos de esta enfermedad en el mundo. Desde marzo de 2023, la Organización puso a disposición en su campus virtual dos cursos gratuitos de autoaprendizaje que buscan contribuir a mejorar la vigilancia y la respuesta a brotes de sarampión.
